Steps and Deadlines
Enroll in 2024 Love Your Life
▢ Enroll in Love Your Life | Begin Earning Incentives
Enrollment includes: (1) online health questionnaire at our Bravo portal; (2) measurement of height, weight, blood pressure, waist measurement, body mass index, body fat percentage and (3) venipuncture lab work including CBC, CMP, Lipids, and optional PSA for qualifying men. An A1C is available on the lab panel for blood glucose results over 100 mg/dL.
Earn up to $400 per year
Earn money for meeting health outcomes or alternatives. Wellness Credits are issued on a quarterly basis for points earned in the business quarter. Wellness Credits, as well as the premium discount, are available for Love Your Life program participants who complete annual fall enrollment by communicated deadlines. Employees with a qualifying benefits eligibility event OR employees hired after 11/1/2023 can enroll in 2024 Love Your Life for the premium discount. This group is not eligible for 2024 Wellness Credits.
▢ Blood Pressure < 140 AND < 90: 50 Points
▢ Glucose < 100 OR A1C < 6.0: 50 Points
▢ LDL Cholesterol < 130: 50 Points
▢ Triglycerides < 150: 50 Points
▢ No Nicotine Use: 50 Points
▢ Track 6,000 Activity Minutes OR 2,000,000 Steps: 50 points
▢ Annual Physical or Well Woman Exam | 100 Points

If you have not already attended a Love Your Life enrollment appointment, contact Employee Wellness. You must first enroll in Love Your Life to have full access at Bravo.

Rewards
Primary employees enrolled on the ARHS medical plan save $100/month on the health insurance premiums and up to $400 per year in Wellness Credits for completing enrollment by communicated deadlines. Wellness Credits are applied quarterly in 2024.

Incentives

- Lipid panel, complete blood count, complete metabolic panel for all;

ARHS Health Plan Discount
Primary covered employees who are enrolled on the health plan and who enroll in Love Your Life by the communicated deadline are eligible to receive a premium discount worth $50.00 per premium deduction ($100 savings per month).Eligible employees who miss or don’t complete fall Love Your Life enrollment are not eligible for 2024 program incentives (premium discount or wellness credits). Employees with a qualifying event for benefits OR employees hired after 11/1/2023 will be invited to enroll in Love Your Life for the premium discount only.
Up to $400 per year in Wellness Credits
Primary employees enrolled on the ARHS Health Plan and Love Your Life can earn up to $400 per year for meeting healthy outcomes. Wellness credits are issued quarterly based on enrollment results and for logging completed activities at your Bravo portal. Wellness Credits are issued for Love Your Life program participants who complete annual fall enrollment. Employees with a qualifying event for benefits OR employees hired after 11/1/2023 will be invited to enroll in Love Your Life for the premium discount only.
