Bravo Access and Account Creation

 

How do I access my Bravo Portal?

  • Use this link https://my.bravowell.com/loveyourlife/sign-in. Use Chrome, Edge, Safari, or Firefox browsers. The link will not work well in Internet Explorer.
  • You can also access your portal by clicking on the Bravo logo (below the banner and above “Achieve Your Health and Wellness Goals”) at the Employee Wellness homepage on the Employee Hub.

How do I register to use Bravo?

  • When you visit the link listed above, click “Create Account.”
  • Input Last Name, Employee ID, and Date of Birth.
    • Employee Wellness uses the information from Lawson to create your eligibility with Bravo.
    • Please use MM/DD/YYYY format for Date of Birth.
    • Still having trouble? Contact Bravo Customer Support at 844-925-2782.

When I try to create my account I receive a notice that states my information doesn’t match their files.

Troubleshooting tips:

  • If your Employee ID is less than four digits, add zeros in front of your number to make it 4 digits.
  • Use your legal name to register.
  • Pay attention to the format that they ask you for when creating your account. For example, your DOB should be listed as MM/DD/YYYY.

Accessing and Completing Your Health Assessment

 

How do I access my Health Assessment?

  • From your Bravo Wellness Checklist homepage, locate your Health Assessment in the big blue box under “Enroll in Love Your Life.” Click “Get Started” under the tab that says “Health Assessment.”
  • You can also locate your Health Assessment in the “Resources” link when you click on the menu in the top left hand corner of your portal.

How can I complete the Health Assessment if I haven’t received my Love Your Life lab results yet?

You can complete your Health Assessment even if you have not received your results, or had your labs drawn yet.

  • Your lab values will populate in your portal automatically after Employee Wellness sends Bravo the lab data.
  • There is a portion of the assessment that asks for your lab values, you can skip those and continue the assessment starting with the questions that ask if your values are generally “normal”, “elevated” or “high.”

Who can see my answers to my Health Assessment?

Only you and our wellness vendor partner Bravo, who administers the Love Your Life website, will have access to your individual Health Assessment report. Employee Wellness, nor any other ARHS employee, does not have access to individual health assessment results.

How will I know I have completed my Health Assessment?

When you have completed your Health Assessment you will receive a wellness score and health age located at the top of your screen. 

 

Logging Your Steps and Exercise Minutes

 

Does my device sync with Bravo or do I need to manually input my steps?

  • Many apps and devices sync with the Bravo wellness portal.
  • Copy and paste this link into Chrome or Edge: https://bravo.livehealthyignite.com/devices
  • Following this link requires you to sign into your Bravo portal.
  • Follow the instructions at the top of the page to properly sync your app or device.

Can I manually track my steps and exercise minutes?

Yes, access your tracker through the blue tab labeled “2 Million Steps/6,000 Exercise Minutes” on your Bravo Wellness Checklist homepage.

  • Click on ‘Track Steps’ or “Record Activity”
  • A new window opens. Click on “Tracker” link at the top of your page.
  • On the new page, you should see a table with options for tracking your nutrition, fitness, weight, sleep, steps, and water intake.
  • Click on either “Fitness” or “Steps” – whichever one you are interested in tracking.
  • Select the correct date for the activity that you want to track and then enter your information and click ‘add’ when finished.

 

Logging Your Physical or Well Woman Exam

 

How do I log my Physical or Well Woman Exam?

  • Log-in to your Bravo Portal.
  • On your Wellness Checklist homepage, find the “Annual Physical/ Well Woman Exam” tab.
  • Click on “Report Completion” and enter the date that you received your exam.

How long do I have to complete my Physical or Well Woman Exam?

In our 2023 program, you will earn points/Wellness Credits for annual physicals completed between 10/1/2022 to 9/30/23.

2024 LYL Enrollment – Key Dates, Details, & Need to Knows

What You Need to Know:

  • 2024 Program Dates: 10/1/2023 – 9/30/2024
  • Love Your Life Eligibility: Primary covered employees who will be enrolling on the 2024 ARHS medical plan
  • Incentives: To be eligible for Premium Discount AND Wellness Credits, you must complete enrollment by the communicated deadlines.
    • Premium Discount: Save $100/month on your medical premiums ($50.00 discount per premium deduction)
    • Wellness Credits: Earn up to $400.00 annually for meeting health goals – more details below.
    • Eligible employees who miss or don’t complete fall Love Your Life enrollment are not eligible for 2024 program incentives.
    • New employees hired after 11/1/2023 or employees with a qualifying event for benefits after 12/1/2023 will be invited to enroll for the premium discount only.

Key Dates and Enrollment Details

  • October: 
    • Review 2024 program rules and annual notice *here*
    • Schedule your enrollment appointment through the online appointment scheduler or request the provider enrollment form through Employee Wellness
  • November:
    • Attend your scheduled appointment for height, weight, body composition, waist circumference, blood pressure measurements AND fasting lab draw (at your appointment).
    • Online health assessment at Bravo
  • Returning in 2024:
    • Meet certain health goals/outcomes to earn up to $400.00 per year in Wellness Credits – details below.
    • Employee Wellness is keeping the reflexive A1C on your lab panel for any fasting blood glucoses > 100 mg/dL. CLICK HERE for more details.
    • If you are unable to attend a Watauga Medical or Cannon Memorial appointment (out of state, travel, etc), you may enroll in 2024 Love Your Life with our Provider Form. Fees/costs may apply (copays, deductibles, coinsurance, etc) – request through Employee Wellness by filling out the appointment request form *here* and selecting “provider enrollment” or emailing lroberts@apprhs.org and return by communicated deadlines.

 

Earn up to $400 in Wellness Credits for meeting health goals

Members who complete the Fall 2023 enrollment for Love Your Life will receive a $50.00 premium discount on any of the ARHS health plans – regardless of the outcome.  For program participants who meet health goals or fulfill alternative standards (by the communicated deadline), additional incentive money can be earned. Up to $400.00 in Wellness Credits can be earned and distributed to Health Savings Accounts, local gyms, or a variety of local retailers. Wellness Credits are distributed quarterly for points earned within that business quarter.

  • Health Goals are based on clinical measures that can be improved with lifestyle changes, medication/physician care, and/or a combination of both.
  • If you are unable to meet the health goal for any reason, an alternative will be given to you. Alternatives include (1) improvement level from previous year (if applicable) and/or (2) Healthcare Provider/PCP goal filed through a waiver/appeal process. For full details, log in to your Bravo portal after 11/1/2023.

How to Earn Wellness Credits in 2024 

▢  Blood Pressure < 140 AND < 90: 50 Points
▢  Glucose < 100 OR A1C < 6.0: 50 Points
▢  LDL Cholesterol < 130: 50 Points
▢  Triglycerides < 150: 50 Points
▢  No Nicotine Use: 50 Points
▢  Track 6,000 Activity Minutes OR 2,000,000 Steps: 50 points
▢  Annual Physical or Well Woman Exam | 100 Points

The Big Picture

According to the Centers for Disease Control and Prevention (CDC), 1 in 5 people in the US have diabetes and do not know it and over 1/3 of US adults have prediabetes and do not know it. Diabetes and pre-diabetes continue to be one of ARHS top condition/disease states. In 2021, 14% of the diagnosed diabetics on the ARHS health plan were newly diagnosed that year.

 

Reflexive A1C added for any fasting blood glucose > 100 mg/dL

When you enroll in Love Your Life, you receive a comprehensive health screening which includes blood pressure, weight, body composition and a multi-panel lab report. Beginning in our 2023 Love Your Life enrollment, the lab panel will now include an A1C measurement at no cost for anyone with a fasting blood glucose greater than 100 mg/dL. Not only with this test better support our diagnosed diabetics within ARHS, but it may also assist healthcare providers with diagnosis of prediabetes or diabetes.

 

According to the National Institute of Diabetes and Digestive and Kidney Diseases:

  •  The A1C is a blood test that provides information about your average levels of blood glucose, also called blood sugar, over the past 3 months.
  • It can be used by your healthcare provider to diagnose Type 2 Diabetes and Prediabetes
  • In some people, a blood glucose test may show diabetes when an A1C test does not. The reverse can also occur—an A1C test may indicate diabetes even though a blood glucose test does not. Because of these differences in test results, health care professionals repeat tests before making a diagnosis.

  • Each visit you make not only improves your fitness, but also enters you in our prize drawing giveaways.
  • Each visit = 1 entry.

    • Not a member? Ask the front desk for a guest pass!
  • At the end of August, ask for a visit summary from the front desk.
    • Scan the summary and email Employee Wellness or send a hard copy to Employee Wellness at our Watauga Medical Center office
    • You must include your Name and Employee ID number
  • Summaries must be received by Wednesday, September 7. Winners announced Thursday, September 8.

Quarter 2 Wellness Challenge

April 1 – 30

We have modified our popular Take 10 for Your Team challenge and present Take 10 for YOU. While Take 10 for Your Team focused on walking breaks, this Challenge encourages you to take a short break at least once a day. You choose how to spend those 10 minutes – whether its stepping outside for a breath of fresh air, grabbing a snack, checking social media, or going to sit somewhere quietly. The choice is yours. Breaks are an important part of your resiliency and health. As the last two years have shown us, overworking can lead to chronic stress and burnout. Breaks are essential in helping you recharge for the remainder of your workday.

How to participate
  • Leaders are registering their departments and encouraging individuals to participate.
  • The Goal: Every participant takes a paid, 10-minute short break away from the work once a day for at least half of the worked days in April.
  • Registered employees will be given an interactive calendar which can be saved the desktop or printed.
  • During the final week of the challenge, participants will enter total number of worked days in April and total number of days that had the 10-minute break.

 

Key Details:
  • Leader Registration: Now through March 23
  • Employee Registration: Now through March 30 – ALL employees can participate
  • Challenge Dates: April 1 – 30
  • Last day to log activities: May 6th

 

Relax and win prizes!
  • All employee participants who complete the challenge will be entered into our relaxation basket drawing.
  • Love Your Life participants who complete the Challenge will earn 50 LYL points in Qtr 2 at Bravo.
  • Completion = 1 short break (10 minutes) was taken on at least HALF of the worked days in April. Participation is logged by the deadline.
