Enrollment: September 6-23
NEW! The only time to enroll in 2018 LYL is September 6-23. Miss enrollment? You will not be eligible for the premium discount on the 2018 health plan for the entire year (excludes new employees and qualifying events). You may still enroll for quarterly wellness credits worth $100.00.
- New Service! Employee Wellness will automatically provide a copy of your LYL lab work to your primary care provider for review in addition to providing a personal copy. It is your responsibility to follow up with your primary care provider for further discussion and treatment if necessary.
- PSA (Prostate Specific Antigen) Test Changes: The PSA test is available for men ages 50-69 with consent only. Look for educational material at the LYL website and at enrollments. Talk with your doctor/provider to see if the PSA test is right for you.

Unable to meet your 2017 health goal?
You may qualify for the same incentives (premium discount) through our Medical Waiver Alternative. No later than November 8, 2017, call (800) 840-6100 or email email@example.com to request the Alternative/Medical Waiver.
Waivers must be returned to Interactive Health by Nov 22, 2017 with no exceptions.
September 6-23, 2017: Love Your Life Open Enrollment
Mid-October, 2017: Health Scores and Goals available from Interactive Health
November 8, 2017: Alternative/Medical Waiver due to Interactive Health
November 22, 2017: Waivers must be returned to Interactive Health
