2024 Rules and Program Details

Eligibility and Enrollment

  1. Appalachian Regional Healthcare System’s Love Your Life (LYL) program is a voluntary wellness program available to employees enrolled on the health plan.
  2. Current employees are invited to enroll every year during the fall LYL enrollment. Outside of annual LYL enrollment, new employees or employees with a benefits qualifying event are invited to join LYL during their benefits eligibility period.
  3. To enroll in LYL, participants must:
    • Review/acknowledge the program rules,
    • Complete the online Health Risk Assessment at the Bravo portal,
    • Have their blood pressure, body composition/weight, and waist circumference measured by Employee Wellness and
    • Have a venipuncture fasting lab draw. The lab order includes CMP, CBC, and a Lipid Panel. An A1C will be administered for anyone with a fasting blood glucose > 100 mg/dL. A PSA for men ages 50-69 (with consent) may also be provided.
  4. As an alternative to onsite enrollment, a provider enrollment form is available for employees who live out of state or for whom attending an enrollment appointment at Watauga Medical Center or Cannon Memorial Hospital is a hardship. The form must be requested through Employee Wellness and returned by communicated deadlines. Health plan expenses may apply (i.e. copays, deductibles, coinsurance, etc).
  5. During Leaves of Absence, including FMLA, employee participants must meet program rules including enrollment. Any exceptions must be approved with the Employee Wellness Manager.
  6. Annual LYL enrollment occurs each fall. Look for notices from Employee Wellness.
  7. Participation in Love Your Life cannot substitute for medical advice, diagnosis, or treatment. Participants acknowledge they will seek the advice/care of their primary care provider(s) for any questions as well as before stopping, starting, or changing any treatment plan.

 

Health Goals and Alternatives

  1. Love Your Life is an outcomes-based program. Incentives under this program are available for employees enrolling in the program and meeting health goals.
  2. Health Goals are based on clinical measures that can be improved with lifestyle changes, medication/physician care, and/or a combination of both. Health Goals focus on no tobacco use, blood pressure, LDL Cholesterol, Triglycerides, Glucose/A1C, exercise/steps goal, and/or an annual well visit with your PCP.
  3. If you are unable, for any reason, to meet enrollment requirements, health goals or complete activities in this program, you may qualify for an opportunity to earn the same incentive through an alternative course of action. Contact Employee Wellness to learn more or for instructions.

 

Program Incentives

  1. Primary covered employees who are enrolled on the ARHS health plan and who participate in LYL are eligible for a premium discount and Wellness Credits. To qualify for these incentives, the employee must complete annual LYL enrollment by the communicated deadline. If all enrollment steps are not completed by the communicated deadline, then the employee is not eligible for program incentives for the entire program year.
  2. New employees hired after 11/1/2023 or employees with a qualifying life event for benefits (outside of annual enrollment) are eligible to enroll in Love Your Life for the premium discount.
  3. ARHS Health Plan discount : $50 discount per premium deduction ($100 savings/month)
  4. Wellness Credits: Earn up to $400 per year based on health outcomes
    • Wellness Credits are earned based on outcomes for meeting individual Health Goals. The goals focus on no tobacco use, blood pressure, LDL Cholesterol, Triglycerides, Glucose/A1C, exercise/fitness goal, and an annual well visit with your PCP.     • Wellness Credits can be spent in a variety of places including HSA deposits, local retailers, and gyms.
    • Wellness Credits, with the exception of HSA deposits, are considered a taxable fringe by the IRS. The taxable income for employees who choose gift cards/account credits at retailers and gyms will be added to the paycheck at the time of reward distribution.
    • Wellness Credits are issued every quarter for new points earned in the previous quarter. Employee Wellness will send email confirmations of reward date/distribution.
    • Once the Wellness Credits are issued for the quarter, employees may not switch or change their election for that quarter.
    • Wellness Credits gift cards/account credits are like cash. It is the employees’ responsibility to keep up with their rewards. Lost or unclaimed gift cards will not be replaced. Employees must follow rules of partnering retailers/gyms for gift card/account credit use and deadlines.
    • Employees who terminate employment with ARHS before the Wellness Credits are issued, are no longer eligible to receive them.

 

Confidentiality

  1. Employee Wellness is required by law to maintain the privacy and security of your personal, protected health information (PHI). Your health information is maintained in strict confidence by ARHS Employee Wellness and its designated affiliates including our program provider, Bravo Wellness, LLC.
  2. The information from Love Your Life, along with any other personal health information you provide to Employee Wellness, is not supplied to your supervisors, nor is it tied to your employment by ARHS or eligibility for health insurance now or in the future.
